Does this pair come with new hardware or attached lower ball joints?

The bushings are installed to connect to the frame via the alignment cams but there are no lower ball joints included. However the lower ball joints available from 1a bolted right up. All parts fit extremely well.

Are the bolt sleeves included? The guy in the video had to cut the stock ones. What about the control arm bolts as well?

These Control arms are very good quality and probably for what you pay a NAPA these would be better. The bushings are standard OE quality and if properly installed, will last many, many years. NOTE: Proper installation requires that when you install these, you must set them at ride height before torquing the cam bolts to 90Ft/lbs. this will prevent the bushing from tearing out when you lower the weight of the vehicle on to them.

Do you have to grease the outside of the bolt sleeves before installation? I know to grease the bolt itself.

If you want to avoid the outside of the cam bolt sleeve rusting to the inside of the new control arm bushing sleeve then I would (and I have) put antisieze on the outside (and inside, why not) of the cam bolt sleeve prior to installing in the control arm bushing. Factory installs dry but in the case of potential exposure to a high rust prone environment where the cam bolt sleeve wont turn in the control arm bushing inner sleeve due to rust you have to sawzall the factory cam bolt sleeve out in order to remove the control arm. Best to prevent that 100k or 10 years down the road. Additionally: The control arm bushing inner sleeve gets clamped by the frame when you tighten the cam bolt, locking the bushing in a specific range of travel, hence the need to tighten the cam bolts when the suspension is under load at ride height, not at full droop. So the antisieze isnt going to see any friction, it just prevents the two metals from rusting together since they dont move for years.
