How hard is this to replace ?

These are easy to replace. The most time consuming issue is getting g the door panel off. After that just look at how the current one is installed before you take it out. There are several videos online to help you with the process. Start to finish should be less than an hour.

Do you have to replace the window guides as well or can you just replace the motor?

This is a manual regulator for models without motors. You can swap out only the motor on your OE part.
