Rotation says clockwise, and my 2004 Civic LX rotates counter-clockwise. Will this be a problem, or is it a typo in your part description???

Do I need to add a ground wire to ground the alternator?

No you do not need to add a ground. Make sure the engine is grounded properly. Remember to check the ground under the air intake. Its hidden and sometimes is bad.
